Overview:
You may change the name on your Driver's License if:
- You have legally changed your name.
- You are a female and recently got married.
Process
Step 1: Applicant submits completed form and documents to cashier.
Step 2: The relevant fee is collected by the cashier and a receipt is issued to the customer.
Step 3: Information is updated. (A new card is printed and given to the customer)

- Duplicate Driver’s License Form
- Proof of name change (i.e. marriage certificate, legal document)
