The public is advised that voters who are unable to submit a claim (Form No.3) during the four-day period of inspection will still be able to vote. 

In order to vote, present the poll clerk with an ID and indicate that you have had a change in your name that was not registered with the Office of the Supervisor of Elections.  Once the Poll Clerk completes the check-in process and directs you to a Presiding Officer, he/she will administer an oath (Form No.28) that you are the voter intended to be reflected on the voters’ list before issuing the voter a ballot.
