The Department of Facilities offers the use of supplies, mainly folding chairs and tables, to government agencies. Ministries, departments, and Office of The Governor are given top priority; however, other government agencies will be considered by availability. To make such request you must complete the [Request for Supplies] form and submit to the Department prior to the event.


Step 1: Customer completes the application form by ensuring all information is filled out and signatures are affixed.
Step 2: Submit completed form to  
Step 3: Await response from the Facilities’ Administration Office 

Additional Notes

  • Forms should be submitted at least 1 week in advance.
  • Priority is given to ministerial functions/events.



Please ensure that all forms are fully completed to avoid delay in processing. Note that where a form is incomplete, an application was denied, or supplies are not available, the Facilities’ Administration Office will contact you in due time.
